Duties:
Responsibilities to Clients
- Ensure the safety and security of clients by means of regularly scheduled checks on the well-being of clients and facility
- Available on an as-needed basis for meeting client’s needs as they arise during the shift
- Assist and participate in various skill-building groups
- Work individually with clients on the development of ADL skills
- Meet regularly with program staff for discussion, guidance, and monitoring of each client
- Maintain accurate and timely entries into the client’s individualized service plan records, as needed and warranted.
- Continue education and training to enhance knowledge and skills
- Act in an ethical and responsible manner in all work related to clients
Responsibilities to Staff
- Consult with and assist other staff in carrying out the day to day activities of the program
- Communicate all pertinent information to staff in a timely manner
- Provide timely, accurate, and complete notes of a shift’s happenings for the staff replacing them on the ensuing shift
- Collaborate with other members of the program staff
Recordkeeping/Documentation Responsibilities
- Document accurately, following agency procedures, completely and in a timely fashion all interactions, important events, progress changes, incidents, accidents, and any other pertinent information about each client
- Complete timely, accurate, and thorough completion of all paperwork assignments
- Follow documentation requirements and appropriate billing procedures
Communication
- Maintain clients’ confidentiality at all times
- Collaborate with other staff in carrying out the duties discussed in addition to all other aspects of the program
- Develop and maintain communication with other staff
- Participate in regularly scheduled and special meetings with other staff to enhance the agency’s effectiveness
- Provide regular, daily reports to the Program Director regarding any client, facility, or programmatic concern
Programmatic Responsibilities
- Willingness to work varied shifts as required
- Willingness and ability to be awake, alert, and responsive to client needs on all shifts
- Assurance of the safety, security, and soundness of the program’s facility/building and belongings
- Provision of complete and accurate inventories of the program’s supplies and inspection of emergency equipment
- Periodic and random checks on the various aspects of the facility’s environment, both internal and external.
- Safeguard of the professional integrity of the program, agency, and their funders in all dealings
- Other duties assigned by the Program Director
Skills Required: Some experience and/or education relating to the program’s clientele, programmatic focus, or related field. Personal skills and/or creativity that can be shared with clients.
Minimum Qualifications: High School Diploma or equivalent, Valid CT Driver’s License
