Population Served - Men on Community Release status from the Department of Correction with mental health issues, ages 18 and older

Program

15 bed residential program 8 - 14 month length of stay individual, group and family counseling daily educational groups including addiction, depression, schizophrenia, anxiety, relapse prevention, healthy relationships, communication and anger management supervised, structured activities of daily living vocational counseling and job search assistance Alcoholics Anonymous, Narcotics Anonymous and Dual Diagnosis meetings medication assessment by a nurse practitioner discharge planning including referrals for suitable housing, financial support, aftercare counseling and continued mental health treatment

Department of Correction counseling or mental health staff Department of Correction Parole and Community Services Unit the client may contact either of the above entities
