Population Served - Individuals with prolonged mental illness, ages 18 and over

Group Home

6 bed community based residential facility individual, group and family counseling daily educational groups focusing on rehabilitation including medication, symptom management, relaxation skills, community resources, recovery, budgeting and financial management, social skills, and relapse prevention supervised, structured activities of daily living discharge planning including referrals for suitable housing, financial support, aftercare counseling and continued mental health treatment

 

Residential Support

20 clients living independently in Connecticut's Northwest corner individual, group and family counseling case management services individualized support and services
